Centerville Office
Our main office is located on North 12th Street in Centerville, the county seat of Appanoose County. The office is easily accessible from Highway 5 and offers convenient on-site parking for all clients.
Parking: Free parking is available in the lot adjacent to the building.
Ottumwa Office
Our Ottumwa location on North Quincy Avenue serves clients in Wapello County and surrounding areas. The office is situated near the Quincy Place Mall area with easy access from Highway 63 and Highway 34.
Parking: Ample free parking is available on-site.
Bloomfield Office
Located on North Dodge in Bloomfield, the Davis County seat, this office serves clients in the eastern part of our service area. It is conveniently accessible from Highway 63 coming from either direction.
Parking: Free street parking and nearby lot parking are available.
Serving Southern Iowa: Paula S. Gordy LISW, LLC proudly serves communities throughout southern Iowa, including Appanoose, Davis, and Wapello counties and surrounding areas. Telehealth services are also available for clients who prefer remote sessions or live outside our immediate service area.

When you call, our staff will help you find a time that works for your schedule and answer any initial questions you may have about services and insurance.
For your first appointment, please bring the following:
- A valid photo ID (driver's license, state ID, or passport)
- Your insurance card (front and back)
- A list of current medications, if applicable
- Any referral paperwork from your primary care provider, if required by your insurance
- Completed intake forms (if provided in advance; otherwise, please arrive 15 minutes early to fill them out)
We recommend arriving a few minutes early to ensure a smooth check-in process.
Yes, we offer telehealth (video therapy) sessions for clients who prefer the convenience of meeting from home or who live outside the immediate area of our offices. Telehealth sessions are conducted through a secure, HIPAA-compliant video platform.
To schedule a telehealth appointment, simply mention your preference when you call our office or select it when submitting the contact form. Our staff will provide you with the details and instructions for connecting to your session.
Our office hours are Monday through Friday, 8:00 AM to 5:00 PM. We are closed on Saturdays, Sundays, and major holidays.
